*$1,500 Hiring Bonus,Additional $1,500 Spanish Speaking Bonus*QUALIFICATIONSEducation & ExperienceA Masters degree in Social Work, Psychology, or Counseling;Clinician I: Registered as an Associate Social Worker (ASW), Associate Marriage and Family Therapist (AMFT), Associate Professional Clinical Counselor (APCC); andClinician II: Registered with CA-BBS as Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) credential; andAt least 1 year of experience providing services to youth and their familiesSpanish speaking requiredESSENTIAL JOB FUNCTIONS Position SpecificFunctions as the lead case coordinator, providing comprehensive, strength-based, culturally competent, flexible, effective, quality mental health services, including psychotherapeutic interventions, to eligible at-risk youth, and families. Provides services based on clinical need and in consultation with supervisor to assigned youth, and families in the home, office, school, or other community based settings.Coordinates and facilitates initial intake meeting with consumers meeting agency and County timelines and expectationsResponsible for the overall assessment, development, and implementation of Assessment Client Plans and Re-Assessment and Re-Authorization Client PlansProvides and documents all Medi-Cal eligible billable and non-billable services, including but not limited to assessment, plan development, group session, collateral, rehabilitation, In Home Based Services (IHBS) case management, Intensive Care Coordination (ICC) and crisis intervention within required timelinesEnsures every family has an individualized safety plan that maximizes the use of natural supports and community based resourcesCoordinates care with any external providers (i.e. psychiatry services, etc)Attends all staff meetings, clinical supervision, and consultation meetings as scheduledProvides services to youth involved in the Child Welfare Services, Juvenile Justice, and Mental Health systems, meeting all County requirements as listed in program scopeParticipates in the agency on-call rotationParticipates in utilization, accreditation, peer review process as assignedResponsible for meeting all outcomes as documented in Employee Development PlanModels and communicates appropriate positive attitudes towards the agencys Mission, Vision, and Values and program outcomesWorks collaboratively with all internal programs, departments, and staff to provide support as neededMaintains positive relationships with consumers and customersCoordinates with other services provided under the Medi-Cal model, acting in collaboration with other partners and ensuring documentation reflects standards that meet or surpass the agency, County Children's Mental Health, and Short-Doyle Medi-Cal standardsAgency SpecificPerforms all duties in a manner consistent with the principles and values of agency, while adhering to applicable professional codes of ethics, the agencys policies and procedures, contractor requirements, and regulatory requirements.Model and communicate appropriate positive attitudes toward the agencys Mission, Vision, and ValuesWork collaboratively with all agency programs and staff to provide support as neededUtilize and maintain calendar with all work-related details in order to manage time effectively and share calendar information with coworkersParticipate in on-going training to expand and develop professional skillsPerform other duties as necessary for the agency, as assignedEmployment At-WillEmployment at the Agency is terminable at-will, which means that employment may beterminated at any time, without cause or reason, by either the employee or the Agency.
