Job Description:
Description Supports PACE Physical Therapists in delivering
treatment and maintenance to preserve or improve participant’s
mobility, relieve pain, and reduce disability for older adults.
ESSENTIAL JOB FUNCTIONS: Therapeutic Intervention & Education
Assists or supervises therapeutic exercises and procedures per PT
direction; Deliver physical modalities (PAMS) as instructed.
Deliver direct physical therapy treatments, including gait
training, transfers, and ADL support. Prepare and maintain a safe,
organized rehab gym/wellness area and treatment equipment.
Observes, document, and report participant’s responses and progress
to PACE Center Manager and Rehabilitation Team Lead. Instructs
participants, families, and staff on home exercise programs (HEP),
body mechanics, and the safe use of durable medical equipment
(DME). Implements a rehabilitation program to promote, restore, or
maintain participant’s optimum level of function. Provides home
health services as needed.Ensure compliance with CMS (PACE)
regulations, APTA Code of Ethics, and California state practice
acts. Follow NEMS PACE philosophy, federal/state regulations, and
APTA ethical/continuing education standards. Participate in Quality
Improvement (QI) initiatives and PACE committees as directed. Be
available for occasional after-hours on-call support for PACE
participants when needed. Maintains required productivity,
documentation, confidentiality, and regulatory compliance. Utilizes
Epic appropriately and efficiently and serves as content and
workflow expert for resolving issues and proposing solutions in
Epic. Performs other job duties as required by Manager/Supervisor.
Qualifications Required: Graduate of a Physical Therapy Assistant
Program approved by the California Board of Physical Therapy.
Licensure or Certification as required by State guidelines and
California Board of Physical Therapy. Valid driver’s license and
ability to perform home/community visits as needed. Current BLS and
First Aid required. Preferred: Minimum one year of experience
working with a frail and/or older adult population At least one
year of clinical experience working with the older adult
population. One year of professional experience in Physical Therapy
in a health care setting (not including internship). Possessing
basic knowledge in all the modalities. Proficiency in EMR (Epic
preferred). LANGUAGE: Must be able to fluently speak, read and
write English. Cantonese and/or Mandarin fluency are highly
preferred. Fluency in other languages in an asset. STATUS: This is
an FLSA Non-exempt position. This is an OSHA high-risk position.
