Job Description Job Description Join a mission-driven,value-based care team as a Primary Care Physician in our PACE (Program ofAll-Inclusive Care for the Elderly) center located in Stockton, CA. We areseeking full time physicians who are passionate about delivering holistic,patient-centered care to older adults through an integrated care model thatsupports clinical autonomy and meaningful relationships with patients. As a Primary Care Physicianin our PACE program, you will provide comprehensive primary care to frailseniors, collaborating with an interdisciplinary team to ensure optimal healthoutcomes. This role includes clinical leadership and offers a uniqueopportunity to make a lasting impact in a supportive, team-based environment. Position Highlights: Schedule: Monday–Friday,8:00 AM – 5:00 PM; no weekends Includes occasional homevisits and rotating on-call coverage (phone only, a few nights/weekend days permonth) PatientLoad: Clinic: 8 patients per day, Field: 4 patients per day Compensation & Benefits: Sign-on Bonus $50,000 Competitive Compensation: $315,900– $386,600 base salary, plus quarterly bonus 17 PTO days 6 sick days 12 paid holidays Medical, dental, and visioninsurance 401(k) with match and equitygrants Malpractice and tailcoverage CME reimbursement Relocation assistance Opportunities to advanceinto a Medical Director role Requirements: Board Certification inInternal Medicine, Family Medicine, Geriatrics, or Emergency Medicine Be eligible to enroll withMedicare and Medi-Cal Meet requirements forCOVID-19 vaccination or provide a declination form prior to employment
