We are seeking to hire several strong RN Case Managers on a full-time basis who have recent experience working in an acute care environment helping acute patients for a trauma level 3 or 4 Hospital. The RN must have excellent computer and documentation skills and a team player attitude which will be keys to success in helping you land this RN Case Management position with this well-known, stable, family-oriented organization. You will be responsible for assessing and discharge planning and the day-to-day management of appropriate and efficient patient care and functions as an advisor to the physician with accountability to escalate cases to the Medical Director if necessary.
